RESPONSE TO BRENDA EVANS – PATIENT CARE QUALITY REVIEW BOARD

Dear Ms. Evans:

RE:  MADDOCKS:  Richard E.

1)    Your File No. 2478 …. and now 13-039 and Michael Orrey’s File # 2495

2)    Update Frequency / Content

3)    Inadequate Response by Patient Care Quality Office

4)    Incorrect Mailing Address – Request for Email-Only Communication

Thank you for your notice of intent to review my concerns pursuant to s. 13 of the Patient Care Quality Review Board Act.  I note the time-frame allows until almost mid-December 2013 for your findings to be made public.  I look forward to those results and trust this lengthy process will be undertaken in good faith … in a transparent, thorough and …. eventually …. meaningful way.

I remain perplexed by the file numbers assigned in this matter.  Ms. Evans, your initial correspondence to me sited file # 2478.  Michael Orrey’s file appears to be # 2495.  And now your latest letter to me sites file # 13-039.  I would appreciate some clarification.

Please advise how frequently I’ll be updated and what information those updates will share.  I will be very interested in those.

I have another concern.  The Patient Care Quality Office’s website states the following commitment from that office:

The Patient Care Quality Office:

“Will provide you with a response to your complaint as well as an explanation of decisions and actions taken as a result of your complaint. (Responses will be collated and completed within 10 business days of the review).”

So Ms. Evans, does the lack of any explanation as to actions taken by the Patient Care Quality Office in their response to me suggest no action has been taken in my case?  I believe the above suggests I am entitled to more of an explanation than the one I just received from the Patient Care Quality Office.  I asked Mary MacKillop if the video tape of my examination by your paramedics was reviewed and if the Williams Lake Hospital records were accessed that fateful evening.  What were the paramedics told?  Thus far I have received no answer to these simple questions.  I wonder if you would explain why please?

I note your office addressed your latest letter to me in Prince George.  I live in Williams Lake.  Fortunately the Canadian Postal Service was able to compensate for this error and I received the envelope nonetheless.  In the future Ms. Evans, in order to avoid any further confusion or delayed documents, I would greatly appreciate that any and all communication be by way of email to rick.maddocks@gmail.com  I can read and write Microsoft Word, Adobe Acrobat, .JPG and other common document formats.
